[pca] EXTERNAL: Proxy

Sounds like the pca-proxy.conf settings need to be checked on the proxy
server. Try adding "debug=1" to that file, then look at the output in
/var/tmp/pca-proxy-debug.txt. This is like turning on the -V flag for the
proxying instance of pca.
