Just to summarize what the others already said:

There are two methods of initiating a reconfiguration boot: Either go to

OBP with "init 0" and run "boot -r", or "touch /reconfigure" while the 
system is still up and reboot with "init 6".

After the installation of certain patches a reconfiguration boot is 
required or recommended. Only some of these patches create /reconfigure 
by themselves.

When using pca to install patches, it will create /reconfigure whenever 
it installs a patch that needs a reconfiguration boot. So if pca tells 
you that a reboot is required/recommended, a "init 6" will always
suffice.
