This is most probably be caused by a corrupt copy of the patchdiag.xref
file.

This file should have a size of 3491140 bytes. If it's smaller, remove it
and re-try. Unfortunately Sun/Oracle doesn't provide a checksum for PCA to
verify valid copies of the xref file.
