[pca] pca and -R

I tend to always run the following before lucreate/lumake when patching using LU and PCA:
pca \
--pattern='[Ll]ive [Uu]pgrade|[Ii]nstall and [Pp]atch [Uu]tilities' \
--install missing
I also have --noreboot and --safe in the config file to avoid any unexpected surprised from the above against a live boot environment.

I've added a paragraph to the LIVE UPGRADE section of the documentation:
pca always installs the patch for the patch installation utilities
first to avoid possible bugs in patchadd. When patching an inactive
BE, this patch should be installed manually to the active BE, as its
patch installation utilities are used even when root=DIR is set.
