[pca] PCA not reporting on patches that may need to be re-installed

I've run into problems where a patch may need to be re-installed if  
someone has added a package after patching.  PCA does not seem to  
catch this.

To test this, I follow the following steps:
1. have a patchdiag.xref that has the following line:
    136882|02|Oct/06/08|R|S| |   
|10|sparc;|SUNWimagick:11.10.0,REV=2005.01.08.05.16;SUNWimagickS:11.10.0,REV=2005.01.08.05.16;SUNWsfman:11.10.0,REV=2005.01.08.05.16;|SunOS 5.10: ImageMagick  
patch
2. ensure that 136882-02 is installed to the packages I have installed  
(SUNWimagick and SUNWsfman) via 'showrev -a' output.
3. pkgrm SUNWimagick.
4. pkgadd SUNWimagick (from S10U6 media).
5. showrev -a | grep 136882-02 (seeing that only SUNWsfman is patched)
6. run pca to find that 136882-02 does not need to be installed.
7. patchadd 136882-02 anyway.
