[pca] patching broke my server!

Is 137111 the only kernel patch during you patching session? There are 
118833, 120011 and 127127 as well. You might want to check the log files 
under /var/sadm/patch/*/log to see if one of the patch failed to install.

Do you have a mirrored root filesystem on that machine, BTW? I think 
I've once read that a broken root mirror might cause similar problems.
